The Connections Readiness Checklist is a required activity for you to reflect and/or engage in structured discussion with other team members around the features needed for meaningful implementation of the Connections Screener. The purpose of this assignment is to objectively consider what items you need to further consider before moving forward. The course facilitator will be providing explicit feedback on your submission in order to help you develop your screening process.
There are 9 items on this checklist and you are required to consider each item. For this assignment to be marked as complete you will need to receive a score of 5 or greater by the course facilitator. Scores will be determined by how you mark your readiness
indicators and the extent to which you consider how you will address any items, particularly those you marked as "no." If you are completing this assignment with others who are also participating in this course as a group activity--be sure to write
the names of each individual who participated in this assignment on the form you submit. Only one team member will need to submit this assignment; however, the facilitator will need to know which course participants were included in order to mark
successful completion on their course page. You will need to complete and submit this assignment and receive feedback (in the form of a grade) before this activity will be marked as completed. Please allow one week for Kim to grade your assignment. If you have any further questions, reach out to Kim or Michele.

Download and complete the word document Updated Connections Readiness Checklist attached below. First, once downloaded, rename the checklist to include the name of your school. After you have completed your checklist, attach (or drag and drop) your completed assignment into "file submissions" then click "save changes." On the next screen select "submit assignment" at the very bottom. Lastly, make sure you select the button to submit your assignment for grading to make sure the facilitator receives your checklist.
